  Date of initial certification*

  Name of the nurse anesthesia educational program completed

  Date nurse anesthesia educational program completed

  Current Recertification dates

*All nurse anesthetists who were initially certified prior to 1985 will only have their recertification status available online.

You will need the nurse anesthetist's Constituent ID number and the last 4 digits of their Social Security Number.

  Requests for Written Verification

Note that some requests are associated with fees that must be paid before any document is sent.

Required fees include:

  State Board of Nursing: Initial Certification

•  For initial certification, candidates will receive automatic notification to TWO of the states.

•  If a candidate requires more than TWO, a $25.00 fee is required for each additional request.

  State Board of Nursing: Recertification: No fee

  Verification to Employer: $25.00 fee

  Verification of Graduation: $25.00 fee

  Nurse Anesthesia Education Program Transcript:

•  $50.00 fee per transcript request
